Links for Life partners deliver services that support people’s social, physical and mental wellbeing needs in Sunderland. The personal information you supply to the Links for Life partners in this form will be processed in accordance with their own General Data Protection Regulation (GDPR) and the Data Protection Act 2018 policies and procedures.

Partners may store and share some relevant personal details such as name, address, age, gender, contact number and any health information that is needed to deliver their services, as well as sharing anonymous data to our funders to demonstrate the programme’s impact. Partners will ask you for explicit consent if they want to share non anonymised details for the above reasons (for example to provide a case study).

The information you provide in a referral form will also be held within the Theseus system which supports Links for Life services. Your form, and the information you have provided, will only be sent to the partner organisation you have chosen.

The Links for Life Coordination Team is responsible for day to day management of the Theseus system – the Team is based within Sunderland City Council, and team members observe the Council’s privacy and confidentiality standards. Anonymised data may be shared with other council departments, and Links for Life Partners to monitor and improve the programme, or for the prevention or detection of crime and fraud where the law allows this.

All Partners have signed up to the Links for Life Members Charter.
